Along the final path you should get your final kills for the Chalice, after you leave the next room with stained glass (the top half of the first room) via the right door. go back to the Organ playing dude and give him the Sheet Music. He reveals the Chalice. Finally, you should have the Chaos Rune from venturing around so go into the room after unlocking it and move the block so the fire is under the Glass Heart. this will cause the Stain Glass Demon to rise in the room you saw him depicted on Stain Glass. He attacks with shards of glass, and physical attacks too if you let him get too close. Avoid his attacks if possible, and when he reveals his heart, pump him full of Crossbow Bolts.
