This 17 year young female presented with large swelling right side of neck. on examination it was a large soft, non tender, cystic swelling with fluctuation positive.FNAC resulted tubercular cold abscess. As the swelling was huge in neck region and chance of spontaneous rupture, incision and drainage planned under local anaesthesia. Patient in supine position with head turned to opposite direction for better exposure. patient part painted with betadine solution and draped. Local anesthesia - lignocaine with adrenaline given . small incision given with No-11 Blade and pus evacuated. we tried to evacuate all pus with that incision, then opening dilated with sinus forceps for better pus drainage.to drain thick pus incision extended little more. after ensuring complete pus drain , betadine soaked guage piece inserted inside the cavity and dressing done . dressing changed after 24 hours and patient advised daily dressing till the huge cavity collapse completely. A course anti tubercular therapy given for complete cure of disease.. + Informações

Giant abbscess being surgically drained. The dr. refused for 7 days prior to draining this to do so claiming "there was nothing to drain" I also had cellulitus going up and around that hip. 6 Antibiotics were tried and nothing worked. In fact just 6 hours prior to this being drained I had an ER dr tell me "there was nothing to drain". This poor old lady came to us with swelling in her left buttock for 10 days.She had history of injection in her buttocks two weeks back. She developed painful swelling and redness in her left gluteal region with difficulty in walking.It was diagnosed as injection abscess left gluteal region which needs incision and drainage under local anesthesia. Patient part painted and drapped.2% Lignocaine with adrenaline was infiltrated around the swelling for proper filed block. I use no-11 blade for stab incision over the swelling at the most fluctuating point of the abscess. You can watch how pus was flowing out from the cavity. The aim is to drain all pus from the abscess cavity. Finger exploration is essential to break all loculi inside the cavity, to know the depth and extend of the cavity and to fascilitate proper drainage of residual pus. after pus evacuation,, the cavity should be irrigated with normal saline and betadine solution. lastly the cavity to be packed with betadine soaked guage pieces. Proper dressing is essential. the dressing to be changed after 24 hours. daily dressing is essential with a good antibiotic coverage. the cavity usually obliterates within a period of seven to ten days.. + Informações

She is a twenty years young female presented with large cystic swelling in anterior aspect of neck. The swelling was of size 6cmx 6cm x5 cm ,tense tender, cystic just above sternal nutch.This was diagnosed as large neck abscess . Repeated aspiration done but the swelling reappeared. So Incision & Drainage planned under local anaesthesia. Patient in supine position. Surgery part painted and draped. Local anaesthesia 2% xylocaine with adrenaline used for field block. After giving local anaesthesia, I used a no 11 blade for stab incision at the most prominent part of the swelling, where skin was thin and fluctuation present. Pus drained form that opening. Little dilatation of opening to be done with artery forceps or sinus forceps. Complete pus drainage to be ensured. Little finger can be introduced inside the pus cavity to ensure proper drainage of pus. The cavity I use to clean with a gauge piece. If necessary curette biopsy can be taken from the wall of the cavity. These wounds usually need daily proper dressing for faster healing.. + Informações

This 17 year young female having a large swelling over right side neck for 4 months.She has History of chronic cough, low grade fever and weight loss of 5 % in last 6 months. On examination found to be a large globular swelling right side neck of size around 8cm x 7 cm x 6 cm occupying right lower part of neck lateral to right sternocleidomastoid muscle and above right clavicle.it has smooth surface, skin over swelling has no redness or any colour change.It is cystic in consistency. Fluctuation test positive.The swelling does not move with deglutition or not becoming prominent with coughing. It was diagnosed as COLD ABSCESS right side neck. FNAC was done to confirm the diagnosis. Now this large cystic swelling contains huge amount of pus in side which needs drainage to give relief to the patient. Incision and drainage is required for evacuation of all pus . Patient also needs a full course of anti tubercular therapy for complete cure. Discussion- Abscess is a collection of pus in side. Cold abscess- as name suggested is cold in nature and non reacting. It does not produce hot or painful abscess as seen in pyogenic abscess.brawney induration, oedema, tenderness abscent.cold abscess is always a sequel of tubercular infection. Commonest site of cold abscess are axilla and cervical region. Tubercular involvement of lymphnode and bone usually give rise to cold abscess.
